What is the GAPP Program?
The Georgia Pediatric Program (GAPP) is a Medicaid-funded initiative by the Georgia Department of Community Health that provides free, in-home skilled nursing and personal care services for medically fragile children and young adults under age 21.
Program Eligibility
-
Age Limit - Must be under 21 years old.
-
Medicaid Status - Must be an active Georgia Medicaid recipient (including via pathways like Katie Beckett).
-
Medical Need - Must be certified by a physician as medically fragile and in need of continuous skilled nursing or personal care support.
Services We Offer Through This Program
-
Personal Care Support - Assistance with daily living activities (bathing, dressing, hygiene).
How to Apply for GAPP
1. Get a physician's order from your Primary Care Physician - Ask your child's doctor to document the medical necessity for in-home care.
2. Choose a GAPP provider: Savannah Specialty Home Care, LLC - Select a Medicaid-enrolled GAPP agency to help coordinate your paperwork.
3. Submit for authorization - the agency submits the care plan to the state for prior authorization (the process typically takes 30 to 90 days).
