Simple Referral Process
A straightforward path from referral to coordinated home support
1. Referral - Complete the referral form and provide available client/contact information.
2. Review - The agency reviews the referral and identifies the appropriate next step.
3. Family / Client Contact - The appropriate contact is reached to discuss needs, preferences, timing, and available support.
4. Care Planning - Services are discussed and coordinated according to the client's needs and applicable care requirements.
5. Start of Services - Once arrangements are confirmed, the agency coordinates the agreed-upon start of care.
6. Follow-Up - Communication with the referral source can be maintained as appropriate and consistent with privacy requirements.
